Elysium Healthcare has a network of 22 sites strategically placed across
England and Wales. We currently have a new, full-time, permanent RMN position
become available at Ty Gwyn Hall.

We offer a 'full-care pathway' solution for commissioners looking to place people with complex conditions right through to transition back into the community.

The hospital is located in Abergavenny, South Wales, within easy access of the M4 and M5 supporting referrals from across the country. The rural setting allows active rehabilitation helping to maintain high levels of interest and engagement.

Our three units provide continuity of care and recovery through a clear step down pathway:

12 bed initial assessment and treatment ward
18 beds ward which provides support to those who are moving forward with their recovery and are able to take on more independence
4 bed step-down service for patients who are nearing discharge

Our Mental Health Nurse role involves:

Adopting a systemic, individual approach to all
patient care plans and communicate the outcomes to the clinical team - both
verbally and in writing, ensuring all CPA and risk assessment documentation is
kept up to date

Ensuring effective communications of any concerns
relating to patient care

Deputising in the absence of the Ward Manager and
their Deputy to manage the staffing and clinical
needs of the ward as required

Supporting and supervising new or
junior staff

Assisting and supporting regional
management in developing and implementing new services

Managing the assessment,
implementation and evaluation of individual patient care plans while promoting
a professional working environment to ensure a high standard of patient care
