Then take a look at what we’ve got to offer: Wellbeing Co-Ordinator The Vale & Cardiff Temporary until April 2018 (this role is funding dependant and is due to be reviewed following a tender process so may extend) Full Time Up to £19,310.75 per annum 35 hrs You will be working with individuals and families to help them develop personal plans for improving their health and wellbeing. This will include helping to overcome any barriers or obstacles using a strengths and asset based approach that is tailored to what matters to them, particularly focussing on increasing immunisation rates and improving behaviours related to tobacco, alcohol and substance use, food and physical activity. You will be working from allocated GP surgeries therefore you should be able to liaise confidently with other health professionals.
